| Registration: |
If you live within
our practice boundary and wish to register with the Practice you
will be required to complete an individual patient registration
form and will be asked to express a preference of GP in accordance
with clause 185. You will be invited to have a New Patient Medical
within 3 months of registration. For information about our appointments

| Results Policy: |
| The surgery will contact patients
only if there is a problem with a result or from
a specimen sent to the Pathology Lab. If you would like information
about your result, then please telephone reception after 10.30pm |

| Comments and Complaints: |
We are pleased to receive comments
and suggestions regarding the surgeries at both Dalston and Wetheral.
We also have a practice complaints procedure - our Practice Manager
will give you further information if requested. You can telephone

There is wheelchair access to both
surgeries. There is a disabled toilet and baby changing facilities
in Dalston Surgery.

Letters: |
| We are happy to provide
copies of referral letters sent to other medical professionals. Please
ask the GP when you are referred. |

| The NHS operates a "zero
tolerance" campaign regarding violent or abusive patients. Such
patients will be removed from the Practice List and offered alternative
